Student Health Surveys/Parent Opt-out Form

Dear Parent/Guardians,
We need your assistance with student and parent surveys that will provide information to
enhance your experience with our schools.
The Georgia Student Health Survey is an anonymous, statewide survey instrument that
identifies school climate issues that impact student achievement. In order to provide a safe and supportive learning environment for your child, the Georgia Department of Education collects survey information from students during the school year. The survey for elementary schools (Grades 3rd-5th ONLY) includes 15 school climate questions and should take no more than 10-15 minutes for students to complete. All student survey data is anonymous and self-reported.
The Protection of Pupil Rights Amendment (PPRA) gives you the right to opt your child out of participating in the survey. If you do not wish for your child to participate in the Georgia Student Health Survey, please click Anne St ES GA Student Health Survey Opt-Out Form 22-23 to complete the opt-out by November 30, 2022. The Georgia Parent Survey was developed to measure parent perceptions of school climate at your child’s school. Your opinion is important to us. Your survey responses are confidential and will be combined with the survey responses of other parents at your child’s school to identify areas in need of improvement. It should take only 10 or 15 minutes to complete the survey.

All surveys will be open through March 17. Please contact your school for a hard copy of the opt-out form or surveys.

Extended Learning Program

Dear Parents,

Anne Street Elementary School Extended Learning Program will begin on November 01, 2022, and end on April 20, 2023. Our goal is to ensure that each student at ASE is afforded every opportunity to grow and succeed academically. Teachers have been diligently evaluating students’ needs based on their summative assessment data to assist with this task. Based on your child’s observational/academic data, they have been invited to join and receive additional instruction in the areas of English Language Arts andor Math. Extended Learning sessions are held on Tuesday and Thursday afternoons, from 3:15 pm to 4: 30 pm. Bus transportation and snacks will be provided. Students not using GSCS bus transportation must be picked up promptly at 4:30 pm. Siblings of students who WILL NOT be attending extended learning should continue with their regular mode of transportation from school unless they have been invited to attend the extended learning program. 

We hope that your child(ren) will be able to participate in this program. Below are the dates each session will be held. This year-long program will ensure that students’ academic needs are met. We are committed to ensuring that all students are receiving the best educational experience possible. Please return this form by Tuesday, October 4, 2022, if you would like for your child to participate.
